Please add a field to specify one primary and perhaps 0 or more secondary CPT values for Tests, Visits, Activities, Treatments and any other items that could possibly have a CPT associated with them. When showing lists of items allow them to be filtered and sorted by the primary CPT value. See attached file

  • Test/Provider "relationship" in Medical History Report

Is there any way to print out a list of Tests with the related doctor/facility where the test was done? I have an appointment with a new doctor who, not only needs a medical history, but will also need information on when and where previous x-rays were done. July 1 - December 31, 2011 = 23.5/mile. http://www.irs.gov/taxpros/article/0,,id=156624,00.html Can you update 2011? In the meantime, I'll calculate it manually from an export.

  • Medications in new release

Since you are working on a new release, there are a couple of bits of info I keep on each medication that would be nice to have as it's own field:Date Filled:Date Expires:Cost/Co-Pay:I rewrite all of these bits into the instructions field so that I have a one page report with each drug and when I need to get it refilled as such:Take one 40mg ...
