• Allergy Coding Errors in CCRs and CCDs

Allergies are not being correctly notated in CCR and CCD records CCR For example, Penicillin (the only medication tested), is translated to Allergy, Unspecified, NEC In Icd9cm_2006. Note that Microsofts HealthVault has this same problem. Interesting! Also, this medical allergy is included in the Problem section. Is it ...

  • RxNorm Coding Problem

RxNorm values are being prefixed with RX1. This is preventing some EMR systems to reject these entries upon import.

  • Re: CCR to CCD Translation Service

Upon further investigation into the NIST validation error, I discovered that HealthFrame allows for a blank entry for the Start Date of a Condition. This causes the file generated with the CCD plug-in to not include an 'effectiveTime' element for the condition.Proposed Solution: Include a null 'effectiveTime' element (e.g. ...

  • CCR to CCD Translation Service

It appears to me that major health care institutions are (or already have started) implementing the import capability using the CCD format. Some do not plan to support the CCR format. The hospital in my area and the associated doctors has started using RelayHealth which allows for the import and export of CCD formatted XML files. ...

  • Re: Incomplete Condition Types

Thanks for noting HealthFrame's use of categorized 'symptoms' and 'conditions'. This does present a problem in that it prevents the notation of a valid diagnostic code to be associated to an office visit. I have not been able to come up with a workaround, which is a legitimate one, to note this. One fix would be ...

  • Re: Change Requests

Yes, I know of the workaround. I commend your design staff for its forethought in adding this feature through-out the product. Rob49

  • Incomplete Condition Types

The lookup of Condition Types is incomplete when compared to the Library Reference Look-up. For example, when using the ''Select Condition'' dialog box to look-up the condition 'pruritus', the dialog only presents two (2) items. When looking up 'pruritus' in the Library Reference Lookup, eight (8) items are presented. This may be a ...
