FYI. This one had me confused for a while…entered a new doctor’s name, address, Medical Info, etc. with a Start Date three weeks in the future for the first office visit. The End Date was blank, but the doctor’s name showed him to be Inactive on the list of Providers>Individual Healthcare Providers. Changing the Start Date to today’s date put ...

Go to Tests>Administered>Specify More Admin Details>During Visit.
If the Visit is to a diagnostic lab for x-ray or blood specimen the auto-enter feature is fine. We normally do not know or care about identifying technician performing the service and leave the ‘By’ field ‘none’.

Test… after clicking ‘Add New’, consider programming the cursor to be flashing in the Item Name entry box. In addition to this example there are other actions in HFE that might be reviewed as well - like Calendar>Add New>Other.
