I have created a summary for you of all pieces of information that HealthFrame requires. I have the information organized per type of data (e.g. visit, test, conditions, etc). For each kind of data, I have marked the pieces that are required.
As you can see, for the most part, the typical information that is required is the 'name' or 'type' for the new record. For instance, a new condition needs to be described, e.g. 'Asthma'; or a new treatment/procedure that is being entered, e.g. 'Appendicitis'.
In some circumstances, the date is important too. If you're entering data about a visit, the date is typically important - even if it is an approximate date (you can always add notes, to remind you that your date was an approximation). Certainly a date is important when entering a calendar event. Note that in some cases, the date is defaulted to the current date ('Default: Today').
There are three other 'special' pieces of information that are required: the provider with whom you are communicating, your relationship to a particular family member and the role of an individual provider (PCP, specialist, etc).
One final point to note is that no information is required for "Birth Information" - though entering your birth date is highly recommended.
