Yes, this is one of the issues I've mentioned in a post or two before, and my work-around was to use self-administered wherever I could, but you are correct -- that isn't really accurate and context is lost as a result. I typically try to recover from that by addressing the fact in the notes, but that has it's issues as well.

Excellent point. This is also true of units when recording test results. As these change over time, or from one lab to another, it would be useful to be able to add to the list in a permanent fashion as you say, rather then typing the ''new'' entry in again and again, risking the potential typo.

That's an excellent point, and I agree. Sounds like you live in a large area, but even in small areas, practitioners do not just practice in one location only, and tracking to which location I went to see the practitioner would be useful.

I agree with your comments about sorting. Being consistent would make the application MUCH easier to use and quicker as well.
