In looking at the Provider contacts, I am finding the the telephone numbers should associated with the locaton rather that directly with the provider. For example, all of my provideres are in two location with different locations which means different days and office hours, telephone numbers and fax numbers.

yes, I can see these features would be of value to me, as then they could be placed on the calendar as items when the quantity and the frequency is known. I also understand that the design flow here requires additional logic and though as to what is common, and what is a more specialized to the individual usage.

I would suggest that the ''end date'' field be moved closer to the label and then the caclculation for the years and months be shown to the right of the calendar icon.
