&lt;P&gt;&lt;FONT face=Arial size=2&gt;1.&amp;nbsp;&amp;nbsp; The Name and phone number of primary physician.&lt;/FONT&gt;&lt;/P&gt;
&lt;P&gt;&lt;FONT face=Arial size=2&gt;2&amp;nbsp;&amp;nbsp;&amp;nbsp;Whether person is a organ donor or not.&lt;/FONT&gt;&lt;/P&gt;
&lt;P&gt;&lt;FONT face=Arial size=2&gt;3&amp;nbsp; &amp;nbsp;Whether person has a healthcare directive.&lt;/FONT&gt;&lt;/P&gt;
&lt;P&gt;&lt;FONT face=Arial size=2&gt;The above things will help tremandously&amp;nbsp;to &amp;nbsp;EMT or ER physican incase patient is unable to give information.&lt;/FONT&gt;&lt;/P&gt;
