&lt;P&gt;1. HealthFrame can be downloaded electronically at the retail cost of US $39.95 or if you wish to purchase a booklet with a physical copy of the CD, you can purchase it at the retail cost of US $49.95 (&lt;A href="http://www.recordsforliving.com/HealthFrame/BuyIt.aspx"&gt;http://www.recordsforliving.com/HealthFrame/BuyIt.aspx&lt;/A&gt;).&lt;/P&gt;
&lt;P&gt;2. Records For Living is located in Massachusetts.&lt;/P&gt;
&lt;P&gt;3. HealthFrame users either enter their personal health record information themselves (based on a combination of memory and/or copies of records obtained from their doctors/hospitals) or they can import electronic copies of their records directly into HealthFrame, as provided by their doctors/hospitals.&amp;nbsp; HealthFrame supports an industry standard called CCR (Continuity of Care Record) and there are a number of electronic health record tools (EHR) - tools used by doctors - which support that standard.&amp;nbsp; For a list of compatible products, you can look at &lt;A href="http://www.centerforhit.org/x1556.xml"&gt;http://www.centerforhit.org/x1556.xml&lt;/A&gt;.&amp;nbsp; You can also ask your doctor if his/her EHR system supports&amp;nbsp;the CCR standard and whether&amp;nbsp;he/she would give you a copy.&amp;nbsp; If so, you can easily import the information into HealthFrame (File / Import menu).&amp;nbsp;&lt;/P&gt;
&lt;P&gt;4. HealthFrame has a major update every 12-18 months.&amp;nbsp; Whenever a major&amp;nbsp; update occurs, IF you choose to upgrade, you'd pay for a portion of the cost.&amp;nbsp;&amp;nbsp; Minor updates are free (for intance, the current version is 2.0.3 and we expect to release 2.1 in about a month or two - when that happens, our paid customers will be able to upgrade for free).&lt;/P&gt;

&lt;P&gt;1)the cost of obtaining the information disk?&lt;/P&gt;
&lt;P&gt;2) where are you located?&lt;/P&gt;
&lt;P&gt;3)Do you contact the physicians or clinics to obtain the information for the disks?&lt;/P&gt;
&lt;P&gt;4) Do you obtain frequently (once a year) and do I have to pay again for updates?&lt;/P&gt;
