Thanks for the screen shot! It's really neat to see our users helping us design the HealthFrame user interface!

Brian recently had commented on the fact that the current support for tracking medical mileage is not smart enough to figure out the difference between mileage expenses (tracked in miles) and other monetary expenses (tracked in $).

As for the mileage. . .have you considered adding an entry box for the number of miles whenever "Mileage" is selected? Actually, even better than that would be to have a completely separate drop-down or check box for mileage and then an entry box for the number of miles.
