    Allergy Coding Errors in CCRs and CCDs

    Allergies are not being correctly notated in CCR and CCD records
    • CCR –For example, Penicillin (the only medication tested), is translated to ‘Allergy, Unspecified, NEC In Icd9cm_2006’.  Note that Microsoft’s HealthVault has this same problem.  Interesting!  Also, this medical allergy is included in the Problem section.  Is it supposed to be in the Alert or Allergies section?  The allergy to Hay Fever is coded correctly in the CCR.
    • CCD – Allergies are not included in the proper designated section (2.16.840.1.113883.10.20.1.18).  Penicillin allergy is included in the 'Problem' section.  This may be due to incorrect CCR creation.  The allergy to Hay Fever is not included in the CCD.
